If you are a person in the United States whose Personally Identifiable Information (PII) and/or Protected Health Information (PHI) was potentially accessible on or about November 28, 2023 as a result of the data incident, including those who were sent a notification from Integris Health, you are eligible to receive a Settlement Class Member Benefit from a Class Action. Excluded from the Settlement Class are: (i) the Judge in this case, and the Judge's family and staff; (ii) Integris' officers, directors, and related companies; and (iii) anyone who validly excludes themselves from the Settlement.
If you are a Class Member and wish to receive Settlement Benefits then you must submit a claim form by December 22, 2025.
Integris will have a Settlement Fund established of $30,000,000.00. The Settlement Fund will first be used to pay (i) court-approved attorneys' fees and costs; (ii) Service Award payments for the Class Representatives; and (iii) the costs of administering the Settlement. The remaining money will be used to pay for the benefits described below to all Settlement Class Members who submit valid claims. All Class Members can enroll in three years of Credit Monitoring and Insurance Services from three credit bureaus. In addition to Credit Monitoring and Insurance Services, Class Members may choose between two cash payment options.
Credit Monitoring and Insurance Services: All Class Members may claim three years of Credit Monitoring Services from the three credit bureaus. These services will include up to $1 million of identity theft insurance.
All Class Members may also submit a claim for one of two cash payment options:
Cash Payment Option 1: Documented Losses. If you incurred actual, documented out-of-pocket losses due to the Data Security Incident, you can get back up to $25,000.00. The losses must have occurred between November 28, 2023 through December 22, 2025. This benefit covers out- of-pocket expenses like:
costs associate with credit monitoring or identity theft insurance purchased directly by the claimant;
costs associated with requesting a credit report;
cost associated with a credit freeze;
costs associated with cancelling a payment card and/or obtaining a replacement payment card;
costs associated with closing a bank account and/or opening a new bank account;
postage, long-distance phone charges, express mail and other incidental expenses;
unrefunded overdraft and/or overdraft protection fees;
unrefunded late and/or missed payment fees and/or charges;
unrefunded fraudulent charges occurring on or after November 28, 2023;
damages and costs associated with any stolen benefits or tax returns;
costs assicated with paying for services meant to remove claimant's PII/PHI from the dark web that was purchased directly by the claimant; and
costs associated with claimant paying a ransom demand to perpetrator of the Data Security Incident.
You need to send proof, like receipts or bank statements, to show how much you spent or lost. You can also send notes or papers you made yourself to explain or support other proof, but those notes or papers alone are not enough to make a valid claim.
Cash Payment Option 2: Pro Rata Cash Payment. Instead of the benefits in Cash Payment Option 1, you may claim a share of the Settlement Fund. This payment is expected to be about $100.00 but may be more or less. The actual amount will depend on the number of valid claims filed. You do not have to provide any proof or explanation to claim this payment. Claims will be subject to review for completeness and validity by the Settlement Administrator.
If a Class Member does not submit a valid and timely Claim Form, the Class Member will release his or her claims without receiving any of the above Settlement benefits.

The deadline to submit a Claim Form online or by email is December 22, 2025. If you are mailing your Claim Form, it must be mailed with a postmark date no later than December 22, 2025.
